OBESITY AND OVERWEIGHT:

Obesity is described as the accumulation of fat in the human body beyond the amount required for the normal body function. This continuous accumulation of fats results weight gain.

Obesity has reached wide range in global, with more than 1 billion adults overweight - at least 300 million of them clinically obese - and is a major contributor to the global burden of chronic disease and disability. Often coexisting in developing countries with under-nutrition, obesity is a complex condition, with serious social and psychological dimensions, affecting virtually all ages and socioeconomic groups.

Increased utilization of more energy-dense, nutrient poor foods with high levels of sugar and saturated fats, combined with reduced physical activity, have leads to obesity rates that have risen three-fold or more since 1980 in some areas of North America, the United Kingdom, Eastern Europe, the Middle East, the Pacific Islands, and China. The obesity widespread is not restricted to industrialized societies; this increase is often faster in developing countries than in the developed world.

Obesity and overweight causes a major risk for serious diet-related chronic diseases, including type 2 diabetes, cardiovascular disease, hypertension and stroke, and certain forms of cancer. The health consequences span from increased risk of premature death, to serious chronic conditions that reduce the overall quality of life. Of especial concern is the increasing incidence of child obesity


BARIATRIC SURGERY:

It is estimated that more than one-third of adults and about 17% of adolescents are obese. The health effects of obesity are myriad, and include the development of type 2 diabetes, hypertension, cardiovascular disease, high blood pressure, and sleep apnoea.

Options for treating obesity have been limited to lifestyle modifications such as dietary changes and exercise as well as the use of weight-loss medications and dietary supplements, many of which have been shown to cause significant health risks of their own. More currently, options for surgical intervention have become more widespread. The term “Bariatric Surgery” defines as a collective group of procedures that involve modifications to the digestive system that promote weight loss; procedures currently performed in U.S. settings include gastric bypass, gastric banding, sleeve gastrostomy, and biliopancreatic diversion (with or without duodenal switch). Most patients are able to undergo these procedures via laparoscopic approach. The choice of procedure primarily depends on 
the severity of obesity, the presence of comorbid conditions, the experience of the surgeon, and the patient’s individual preferences or other contraindications.

ENDOCRINOLOGY:

Endocrine systems of the body play an important and pervasive role in both the short-and long-term regulation of metabolic processes. Nutritional, behavioural, and reproductive processes are intricately regulated by endocrine systems, as are growth (including bone growth/remodelling), gut, cardiovascular, and kidney function and responses to all forms of stress. Disorders of the endocrine systems, involving both overactive and underactive hormone secretion, result inevitably in disease, the effects of which may extend to many different organs and functions and are often debilitating or life-threatening. Viewed from this general point of view, the threat posed from environmental chemicals with endocrine activity (either agonist or antagonistic) is potentially serious. However, the fact that humans and wildlife are exposed to such chemicals does not necessarily mean that clinically manifest disturbance of the relevant endocrine system will result, because much be dependent on the level and duration of exposure and on the timing of exposure.
